Dual Diagnosis
The Active Outreach Team’s work is based on ‘Assertive Outreach’ and ‘Strengths’ models of care - the work is flexible, longer term and more intensive.

There are six full-time workers, most of them Registered Mental Nurses or Social Workers. The team is funded primarily by Norfolk Social Services Department and Norwich PCT. One member of the team is a specialist Dual Diagnosis worker who works with individuals who have substance abuse issues, and provides training for the team and a link to Drug & Alcohol Services.

Referrals for this service are accepted from Norwich Mental Health Social Workers and Community Mental Health Teams, and should be sent to:

Criteria for Support

All clients referred for support by the Active Outreach Team will be experiencing severe and enduring loss of mental health that impacts on their ability to sustain themselves in the community.

They will also meet at least one of the following criteria:

At risk from severe self-neglect
Disengaged from statutory services
Disengaged from family and community network
Would benefit from a less formal environment
At severe risk of relapse (where might this person be in X years time if a service isn’t offered?)
Chaotic lifestyle
Mental health has impacted on day-to-day living skills
At risk in the community, either through their own behaviour, or severe harassment from others

The Active Outreach Team can also offer a helpful service to:
Parents or carers
Clients experiencing their first psychotic episode, when this is accompanied by a severe disintegration of life and social skills

What does the Active Outreach Team do?
Provides an ongoing assessment through practical interventions
Provides a proactive rather than reactive service based on client needs and wishes
Attempts to engage client and significant others in a collaborative relationship. (Other services, family, neighbours.)
Offers relapse prevention by identifying ‘prodromal’ signs, and providing more intensive support at times of increased vulnerability
Helps to diminish chaos and increase stability
Makes the client ‘visible’ to other services
Explores options and increases choices for clients both within traditional services and the wider community
All work undertaken by the Active Outreach Team is rooted in evidence based practice for this type of support.
