HART
The HART service comprises of Julian Housing’s Oak House Team in partnership with Norfolk Mental Health Care Trust’s Hostels Rehab Team. Broadland Housing Association is the landlord of Oak House and Sorrel House. Its aim is to give individuals with enduring mental health issues a supported route into appropriate accommodation.

Individuals who feel ready to concentrate on working towards independence or identifying the most suitable housing options, are offered three types of supported accommodation from where to work towards their own housing goal:

1 Oak House
Oak House, located in the Norwich suburb of Bowthorpe, offers a twenty- four hour supported housing environment for 9 individuals who are ready to think about their longer term housing options and who require a full housing and strengths assessment. The expected duration of stay being up to a year, based on individual need.

2 Sorrell House
At Sorrell House, also located in Bowthorpe, we manage 14 studio flats offering individuals a greater degree of independence than at Oak House. Individuals are offered outreach housing support from Oak House which is nearby, and telephone support (available between 9am and 9pm, seven days a week). Tenants will also receive housing management support from Broadland Housing Association via a housing officer and an on-site neighbourhood warden. The expected length of stay is about 6 months, depending on individual need.
What is meant by support at Oak House and Sorrell House?

Individuals at Oak House and Sorrell House are offered practical advice and support to assist them to develop or relearn skills related to moving on to appropriate accommodation. Support is mainly concentrated on daily living and domestic skills, budgeting and finance, assistance with accessing social activities and contacts, liaising with the individual and their wider care team to monitor mental health and emotional well-being, and assistance in identifying and applying for appropriate housing options.


3 Continuing Support Hostels

The HART service is able to access places in the in-patient hostels.

What is meant by support at the in-patient hostels?

Registered and non-registered nursing staff support residents of the in-patient hostels. Individuals accepted for the service are not classed as discharged from hospital, they are in-patients. Individuals remain under the care of their consultant.


What are the referral criteria?
Applicants will, in general, be expected to meet the following criteria:
Able to control any alcohol dependency / actively working towards eliminating illegal drug dependency
Over 18 years of age and up to 64
Able to share with others in a mixed group setting (Oak House)
Able to share with people from a variety of backgrounds, ages and cultures (Oak House)
Able to manage on a day-to-day basis with background practical support, the level of which may vary according to needs
Able to budget for themselves, with support
Able to negotiate stairs (where accommodation is not fully accessible) and manage simple household tasks

Decisions on applications for housing or support services will also take into account the following:
Balance of needs in the project, where appropriate
Risks factors, such as incidents of violence or arson

Who can refer to the HART service?

Referrals are accepted from mental health professionals who have clients with on going mental health and resettlement needs, and who fit the eligibility criteria detailed above.

How do I refer an individual to the service?
There is one point of referral to the HART service. Mental health professionals who are interested in referring clients are asked to contact HART (based at Oak House).
Involvement of the current Care Coordinator

Referrers will be made aware whilst progressing through this process that there will be an expectation for the individual’s current Care Co-ordinator, to attend housing reviews held every 4 - 6 weeks to assist them to follow the process with their client.
